If saline implant ruptures, how quickly do you need to get it removed?
My saline implant is leaking! How quickly do I need to fix it?
Jan 5, 2021
Breathe.

Saline implants are filled with saline. When they leak, the saline just gets absorbed by the body. Sometimes they leak quickly, sometimes slowly and intermittently. "Rupture" sounds so violent. It really is quite boring. It just gets smaller and more wrinkly over time. When the skin becomes "unstretched," it may be more sensitive. This is NOT anything bad happening. It is just the nerves in the skin becoming unstretched, which can make them sore/tingly/itchy.

There is no urgency...

How bad would the scarring be if I got my implants removed to go smaller with an areola reduction
Removing implants and doing a lift. Do the vertical short scar lift.
Apr 11, 2020
You have had many great answers here, so I will be brief. When you are droopy and have larger areola, doing a breast lift is a great idea, especially when you are so young. There are two basic scars from doing lifts: one that looks like an anchor, and one that looks like a lollipop. Please find someone who does a vertical lift. I think it is a better lift, longer lasting, better shape, and a shorter scar. You are young. You will have these scars for the rest of your life. Make them as small...
How will I achieve this look after explant?
How will I look after removing breast implants?
Apr 11, 2020
Removing breast implants will change your breasts.Exactly how you look varies on many factors:How long were the implants in?How old are you?Have you gone through pregnancies, breastfeeding, or weight changes? What is your skin quality? How stretchy is it? What is the natural volume of your breasts (Those of us in practice for a long time have gotten pretty good at estimating this. I also have Vectra 3D at my office which helps estimate size)Do you have saline implants? (I do pre surgical ...
Surgeon persists I wear underwire post BR. Is this normal?
Underwire bra after breast reduction? (Yes, I too recommend it for my patients)
Dec 14, 2018
As other doctors have stated, every doctor is different, and you should defer to your doctor's recommendation.I love underwire bras after breast reduction. Why?I do the short scar technique, so an underwire isn't running along the scar line (like it does on an anchor scar).I think underwire bras are just better supportive bras. You will swell to fill whatever shape you put your breast into right after surgery.  An underwire helps make sure you are supported in the right places and shape. It ...
Why are there so many mixed opinions on capsule removal?
do you need to remove the capsule if you remove breast implants?
Mar 9, 2018
My short answer: yes.I was surprised to see so many differing opinions on this.  Everyone with an implant forms a capsule. I did a breast fellowship after my plastic surgical training, and do a lot of breast revisions in my practice. When I do a revision, I always see a capsule.  Sometimes it is thick and calcified, sometimes it is hard, sometimes it is thin and flimsy. It doesn't always correlate to how things felt on the outside. If you leave the capsule in, the space where your implant r...
Is it better to remove implants through the areola which is my original scar, or underneath the br
should you use the same scar to remove your breast implants?
Mar 9, 2018
This is not a straight answer, as there are pros and cons.
First, I would recommend removing your capsule.  I saw some doctors said just to remove the implant, but if you do not remove the capsule, the tissue isn't "sticky" and the space the implant was in could persist.
PRO of using same scar: no new scar. easy access to the pocket for implant removal. The only real NEGATIVE:  I see is that occasionally you can get a puckering/indentation along that scar as there is no implant behind the s...
Do I need to get implant removed & breast lift or just removal?
do i need a lift after breast implant removal?
Mar 9, 2018
Super tricky to answer online, because we can't see what you look like.  Some women look okay after implants are removed, and they do not need a lift.  Others would look droopy.  How can you tell?Without seeing you, some simple thoughts:1. how old are you? (age 24 indicates you are young, so likely skin tone better.)2. have you had anything which has stretched out your skin? ( pregnancy, breastfeeding, major weight changes).  All of these things indicate your skin won't "bounce back" as wel...
Which product is best for cheek augmentation?
am i a candidate for fillers?
Jun 28, 2017
When you hit your 40s, you really start to see the loss of facial volume.  It is sneaky, and usually people come in saying "I look tired."  The issue is really one of volume loss, particularly in the cheek area.  Fillers are a great way to improve this.  I generally use HA fillers like Juvederm, Restylane, Restylane Lyft, or Voluma.  It is an easy, in office procedure. When people see what it does, if they want something longer lasting, I love to do fat grafting. I would avoid Sculptra...
Do nano fat injections work for acne scars?
nanofat to scars
Jun 28, 2017
I am a fan of fat grafting to scars to try to help soften them.  It isn't magic.  It doesn't work quickly.  But I absolutely see improvement in the quality of skin and the depth and feel of scars.  Nanofat is when you break up the fat- you are really focusing on the stem cells in the fat- and inject with a sharp needle into the skin itself.Absolutely worth trying.  Don't expect a quick response.  When it does work, it takes months to see the improvement.
Does it matter what equipment or method the surgeon uses for fat grafting retention?
what is the best method for fat grafting survival?
Jun 28, 2017
Ah. It is a great question, and one which we cannot answer yet.  There have been multiple studies which collated all the studies out there about the best way to harvest, process, and inject the fat to improve fat survival. What did they find? They can't tell yet.  In general, there is an ideal fat globule size.  Concentrating the fat with filtering or centrifuge has been shown in some studies to improve survival.  Spreading out the injection when placing the fat is better.  A great study...
27, no kids. Would I qualify for a mommy makeover or would I have to lose weight before?
mommy makeovers are not just for moms.
Jun 28, 2017
Mommy makeover is a general term for doing surgery to the breast (lift, reduction, augmentation) mixed with body surgery (liposuction, tummy tuck). The closer you are to your ideal weight, the better.  Why?If you lose weight after, you will loosen your breast lift or abdominal skin.Breasts and body are all about proportion.  If you lose 10 pounds does is come off equally? Does more come off your belly or your breast?Surgical risks are all better with BMI in the normal range.  Lower rate...
How likely is it that I can get my health insurance to cover my breast reduction? I'm a size 36E a
will insurance cover my breast reduction?
Jun 28, 2017
We, your plastic surgeon, do not get to decide.Every insurance company is different, every plan is different.  I have specialized in breast surgery for 17 years.  When I started my practice most of my reductions were covered by insurance.  Now few are. Most adhere to the Schnur scale, which mandates a certain number of grams be removed to get coverage.  I have had patients with multiple other symptoms- jaw pain, shoulder issues, MRI changes to the neck- who were not covered because we did no...
Small circular opening 8 weeks post breast lift. Should I be concerned?
spitting a stitch after surgery
Jun 28, 2017
What is spitting a stitch? When we do surgery, we put in hundreds of sutures.  The deep sutures are usually dissolving sutures, which your body dissolves by an inflammatory process which breaks it down.  When this happens, sometimes you body feels lazy and instead tries to "spit" the suture through the skin to get rid of it.  It will look like a zit frequently- a little hard or red bump.  This then comes to a head, and a little whisker may be felt.  Sometimes there is a little white disch...
Just to show my size. Am I a good candidate?
are you a good breast reduction candidate?
Jun 28, 2017
If you feel your breasts are too big, then you likely are a candidate for breast reduction.  I think a reduction is a great surgery particularly when your breast size is impairing your life.  Do you have a hard time finding clothing? Can you not exercise due to the bounce? Do you avoid pool parties because of how you look?Timing of surgery, how much to remove, and other issues should be discussed with your doctor. The fact you are calling yourself "topheavy tiff" indicates to me you want to...
My implants are leaking silicone and need to come out, can I get new ones at the age of 71?
can i replace implants when i am 71 years old?
Jun 28, 2017
Age is a relative thing.  Some 71 year olds are hiking mountains, some cannot walk down the block.  You have to look at your overall health.  What other medical issues do you have? Are you actively smoking? Seeing your general doctor for a full physical is step one before deciding on surgery.  Safety is paramount with elective cosmetic procedures. If you are cleared for surgery, you should stop smoking for one month ahead of time and one month after.  If you have a breast implant, if you ha...
