Sub pectoral muscle implant placement requires the creation of a pocket that “fits the implant.” When the pocket is too large or the implant is too wide, either from implant displacement from stretching or surgical pocket creation, this is solvable with further surgery as you appear to be aware. Successful improvement of the corners of the mouth can be achieved with the use of Botox and fillers such as Vobella which I have come to prefer during the last 2 years. Corner lifting is possilbe as well as the use of other fillers which is the choice made after exam. Facelifting in your case is not indicated at this time and the addition of fillers to your cheeks would not influence your mouth corner problem although it would improve nasolabial folds which are not of concern in your photos.

Liposuction which removes fat also creates looser skin ie more laxity. This is why liposuction is done during,i.e., before skin excision of abdominplasty which removes more skin and gives you a better result as skin redrapes better and does not leave any residual skin behind. If as you appear to have, you have a great deal of skin of your lower back and flanks, then consider a circumferential lift which allows excellent reshaping of your mid torso.
