Doctor’s answers

Question
Answer Header & Date
Answer Snippet
Was told I need a reduction. Would Botox be able to correct the chin ptosis?
Chin
Aug 20, 2020
The chin should lay just posterior to the bottom lip on the profile view. Your chin certainly extends far anterior to this bringing disharmony to your face. Botox will not correct this. A chin reduction of the actual bone is what you need to bring more harmony to your face.
Moderate plus or high profile? Hoping to get between 200cc-235cc.
Implants
Aug 20, 2020
Your implants are designed to sit behind the breast. The surgeon should begin by measuring the base width of your breast and pick implants based on this base width. Then, differing volumes can be accomplished by adding or taking away projection. Many surgeons have implants that you can try on in the office to give you at least some impression of what differing sizes would look like on you.
Buccal fat removal or fillers to help with jowling/no cheekbone definition (33 y/o)? I am 5’5
Buccal fat
Aug 20, 2020
Removal of your buccal fat will give you a thinner cheek area, not jaw line. In my opinion, I would first try malar augmentation with filler. This would bring better balance to your face and relatively speaking has little cost and downtime. More permanent solutions including midface implants can then be considered
Can my breast ever look normal without implants?
Reduction
Aug 20, 2020
Based on the picture, it appears that you need a reduction, which also includes a lift. I certainly would not advise implants given then large size of your natural breasts
Can I get a rhinoplasty without sedation/anesthesia?
Rhinoplasty
Aug 20, 2020
What you are describing will need general anesthesia. Not only is this operation painful, it is paramount that you remain still during this procedure, which cannot be guaranteed without general anesthesia.
Lower face ageing. Any ideas what may help?
Lower face
Aug 20, 2020
This is not an adequate picture to give a full assessment. You would need to send full pictures of your face in the frontal, oblique, and lateral views. From this picture you can see that you have prominent marionette lines. This could be addressed with filler. Normally when people are concerned about their lower face, a neck lift with or without a facelift is considered.
Removal of hump with slight shortening of tip. Easy or hard to achieve?
Rhinoplasty
Aug 19, 2020
While no rhinoplasties are easy, your goals can be obtained with a routine rhinoplasty. It is important that you go to a surgeon that you are comfortable with that you feel understands your goals and can achieve them.
Would a brow lift raise my brows/decrease hooding without raising my hairline?
Brow lift
Aug 19, 2020
A traditional brow lift using a coronal incision would elevate your hairline, giving you a larger forehead. A way to avoid this is to perform a direct brow lift (incision right at the brow) of just the lateral brow. This would not elevate the hairline and would leave the scar hidden at your brow level. Of note, raising the medial eyebrow is actually the look of a person in their 50s. Women on their 20s have low medial and central brows with an elevated lateral peak. This should be your goal….
What is the best suggestion to fix my deformed breast after implant removal?
Breast contracture
Aug 19, 2020
I wonder if the surgeon also removed the capsule along with your breast implant. Leaving the scarred capsule behind can still leave the breast with an altered look, so if this was not done, this would be my suggestion. Then, I would recommend a mastopexy to lift the nipple and remove excess skin. Depending on your desired size, you could do fat grafting for volume augmentation at the time of this mastopexy
Where is the Botox injection site for frown lines and how can I lift eyebrows?
Botox
Aug 19, 2020
I assume by frown lines, you mean vertical glabellar lines. The muscles that control this are the bilateral corrugators and the midline procerus muscle. These are thus the Botox targets. This should not lower your eyebrows as you have stated. There is a risk of causing drooping of the eyelid with this injection if not done accurately. To raise the eyebrow, injectors should actually inject under the brow at the orbicularis muscles. This will allow for a slight brow elevation as the…
What kind of breast lift would give me the best results? Would implants with breast lift make
Breast reduction
Aug 19, 2020
Given the size of your breasts, I would offer you a reduction. This entails removing excess skin to give you a lift but also removing some breast tissue to give you a smaller, better shaped breast. This would also help with inevitable neck/shoulder/back pain that you have had or most certainly will have throughout your lifetime because of your heavy, pendulous breasts.
Should I wait to finish having kids before I get a breast augmentation or is it okay to go ahe
Breast augmentation
Aug 18, 2020
Obviously, a picture would be helpful to answer your question. With pregnancy and breast feeding, your breasts enlarge and then shrink, however the skin envelope may not contract. This can result in ptotic breasts with excess skin and stretch marks. This usually requires a mastopexy with or without an augmentation. Given how much your breasts changed with the first pregnancy, it is possible the same will happen with subsequent pregnancies. Therefore, I would wait until you’re done having k…
Do I need a full lift (anchor lift)?
Breast lift
Aug 18, 2020
It is hard to answer your question without examining you. The anchor addition (the horizontal incision) allows for removal of skin in the horizontal dimension, which is not accomplished with a vertical incision. My assumption is to get an ideal result, you would need at least a small horizontal incision, however this incision is hidden under the breast which is difficult to see.
What are my options for plus size surgery? Am I a good candidate?
Mommy makeover
Aug 18, 2020
Your current BMI is >40 and is not in the healthy range. Not only does operating on you at this current weight increase your complication profile, it also makes it very difficult to get an ideal cosmetic result. You should get to a healthy, steady weight before undergoing any cosmetic surgery.
Please advise on achieving natural results to suit my frame and to correct asymmetry. Options?
Breast augmentation
Aug 18, 2020
You appear to be an ideal candidate for a breast augmentation. Round smooth shell implants are what you need (textured should not be placed at this time). You need a surgeon to measure the base width of your breast and then you can discuss size based on projection of implants.
