I think you need to have a full wise pattern or anchor breast lift to remove you excess skin. You currnetly have realtivel small implants but if they made you a DDD it tells me that you have a fairly significant amount of breat tissue. As part of hyour lift yo will get some reductin of you breat size. Ading an implant will give you better upper pole fullness, I would be conservative and consider a low or at most a moderate profile implant under 300 cc's. I would lean towards a low profile...

I agree that your nipple will look too low on your implant when you are out of your clothes. The amount that your nipple needs to be elevated is enough that I woudl recommend a lollipop lift. I think you should do the lift and augmentation at the same time. When lifting a breast an augmenting it at the same time there are things that will hppen as you heal. Yes, you can have a tummy tuck safely with your history. Since you have had a DVT I would recommend starting lovenox after surgery. There are many risk factors for developing DVT. Your Plastic Surgeon should go over these with you to determine if you have any other risk factors. It is possible but unlikely. Many patients who have lived with large breasts want to be as small as possible. They also often have specific cup sizes they want to be. I think you will get a good resutl with this plan. It will still be a fairly long scar but you won't have a scar around the belly button. My only concern is how well the skin above your belly button (where the stretchmarks are) will contract. This might look a little wrinkly after liposuction. If you want maxinal skin tightening thei would consider a full tummy tuck. Talk with your surgeon and make sure your are clear about what your goals are.

This is done to allow for more skin removal and hence produce more tightening of the skin then can be with a traditional tummy tuck. This wold be typicaly done in a patent who has lost a lot of weight and has beenleft with an extreme amountof excess skin. Liposuction is not likely to give you the result you want and I would not reccommend it. A tummy tuck might be an option depending on what your goals are. If you have a lot of overhanging, heavy lower abdominal skin this can be removed with a tummy tuck. What the tummy tuck won't accomplish is making you look thin. I don’t think you will be able to achieve the look you want without having an implant. A vertical lift or inverted T  ( anchor ) lift will tighten the skin and elevate your nipple. It will not give you the upper pole fullness you desire. Fat grafting can help but I doubt you will achieve as much fullness as you would want.

This is a very difficult question to answer without actually talking to and examining you. I think you have very nice, natural breast shape and breasts that are already large. Implants don't really lift the breasts...they make them bigger and more full in the upper pole. It looks like you have enough breast tissue to have the implant placed on top of the muscle (pre-pectoral). It is suggested that you have at least 2cm of tissue thickness, by pinch, in the upper part of your breast. Without examinng you I can't say this for sure. The benfits of pre-pectoral placement are less post-op pain (usually), no breast movement with muscle contraction and a more rounded fuller uppper breast. It is hard to say from the picture if the grafts you dislodged were viable grafts. Typically by one week the grafts, if they survived the transplantation process, are difficult to dislodge. There will always be some number of grafts that don’t make it. In some very rare cases, all of them can die. Despite this your 10 day picture looks encouraging.

A Mirena IUD releases  the hormone progestin. Unlike estrogen, progestin has not been shown definitively ( ther oare very few studies ) to increase your risk of DVT.  Women who have had a DVT ( bloood clot ) or PE usually are not put back on estrogen based contraceptives but may be placed on progestin contraceptives. There are alway trade offs to having a particular surgery. WIth a tummy tuck the big trade-offs are scars ( the long one in your lower abdomen and one around your belly button).
