Breast implants in New York City themselves, either saline or silicone, pose no health risks to your body-even if they rupture.

There has been significant research and studies performed on this topic over the last several decades and the FDA has approved both saline and silicone gel implants for cosmetic breast augmentation.

The implants do not last a lifetime and you will likely require another surgery in the future. The implants can become infected, which is extremely rare.

The implants can rupture, which is in the 1-10% range over ten years depending on silicone vs saline.

The implants can develop capsular contracture, which is a hard type of scar tissue which forms around the implant, sometimes causing painful symptoms requiring their replacement.

The good news is that your situation and question is not unique. Many t hin, athletic women seek breast augmentation. The answer depends on what risks you are willing to tolerate. On top of the muscle, the implant will not move appreciably with muscle contraction.

I see that as the only advantage. Under the muscle, there is a significantly lower risk of encapsulation, better mammograms and less visibility of the implant (what many call the fake look). You are less likely to see rippling of the implant under the muscle.

Despite our best efforts, the implant will move with contraction of the muscle in some patients. Fortunately, it doesn’t happen often. My bias is obviously for under the muscle with a dual plane approach.

It has worked well for many femal athletes.

Personally I would strongly recommend that you consider a breast lift in conjunction with placement of breast implants in New York City. A breast enlargement alone will help your figuring clothing but a breast lift will improve the contour of your breast and their appearance when undressed.

The trade-off of the surgical incisions is generally worthwhile but it becomes a personal choice. Discuss it carefully with your plastic surgeon and good luck.

Breast Asymmetry

Breast asymmetry is very common, especially if your breasts were not symmetric before your procedure. I suggest waiting until you are fully healed before addressing your concerns with your surgeon.

Wait at least six months

I typically allow six months of resolution from your pregnancy state back to your prepregnancy state. This time will allow all the changes become stable.

Breast Implant size

You are on the right track. As a general rule, the impants get larger by increasing diameter of the implant more than projection. A 36″ chest is not terribly large, but still will need a larger implant.

The most likely women to get stretch marks after breast implants are young (usually under 21), and have very firm tight skin of the breasts. if the implant is very large and the skin is very tight, rapid expansion with placement of an implant will be more likely to result in stretch marks.

placeing the implant under the muscle can be protective and reduce the stress on the skin to some degree. if you think about stretch marks as “cracks” in the deep and firm layer of the skin (the dermis) that leaves the thin epidermis thinned out trying to keep the cracks “covered”, it makes more sense.

pregnancy, rapid weight gain, and puberty (with high hormone levels and rapid development of hips and breasts) are much more likely to cause stretch marks than implants. your plastic surgeon will be able to give you an idea as to whether your skin condition and the phase of life you are in make your risk of stretch marks higher than average.

stretch marks are, however, generally not seen often as a result of placing an implant in the breast for breast augmentation. a small benefit of breast implants for women who already have stretch marks, is that stretch marks generally look much better when over-stretched and deflated breast skin is filled up with an implant!

Recommended Waiting Period between Breast Feeding and Breast Augmentation

The consideration has to do with the fact that the breast milk ducts are not totally sterile and that placing implants through this environment may be associated with a slightly higher rate of capsular contracture and implant infection.

The generally recommended waiting period between end of breast feeding and a breast augmentation is between 3 and 6 months. At 9 months, you should be able to go ahead and have the procedure.

Breast Augmentation Above or Below the Muscle

There are many advantages to having a breast augmentation below the muscle.

1. It looks more natural especially on thin patients or those with little breast tissue as the muscle is one more layer hiding the implant.

2. When you get mammograms, the detection of cancer is the same as without implants when they take an extra view as the implant is totally below all the breast tissue.

3. It decreases the risk of capsular contracture.

The disadvantages are that you may move the implants slightly when you use your pectoralis muscles, and you cannot do pectoralis work-outs for 3 months after augmentation to allow the implant to settle down into the pocket.

Which incision

Recent medical studies show that the infra-mammary incision leads to a lesser risk of capsular contracture.

How long implants last

The amount of time an implant will last will vary from person to person. Just because they are 10 years old or more does not mean they need to be removed unless they are ruptured .

Does 100cc or 200cc equal 1 cup size?

There are general thoughts about volume and cup size, and for many it takes at least 100-150 cc for a visible difference and 150-200cc for one cup size. You however should forget numbers and focus on what you wish to look like and what you wish to wear.

The number on the sizer is less important than how you feel with the look and how you feel with clothing.

Transxillary vs. Inframammary Approach for Breast Augmentation

While there is a role for the transaxillary approach in breast augmentation, every perception you have about the benefits of the inframammary or the sub inframmary approach is correct. It is far from old school and is still the workhorse of incisions for breast augmentation in New York City.

Breast Implant size

Your choice of implant size would depend on what you want to achieve. With your small stature – 4 ft 10in a modest size implant would give you a great result. You would need a full consultation to check your breasts, measurements and I routinely perform 3D imaging so that you can see what different size implants will look like.

Options for breast implant placement

By options for placement there are several considerations-the two that most people think about are location of the incision and then position of the implant in relation to the muscle. Incision location is usually decided based on surgeon and patient preference.

In a patient with a well defined breast crease (infra-mammary fold) I personally prefer to place the incision there because it is well-hidden. However, others prefer different approaches. As far as implant location, if you have very thin tissues I almost always place the implant under the muscle; it gives you extra coverage over the upper inner half of the implant where, in thin patients, the edge of the implant is likely to show.

As far as on top of the muscle versus below the muscle thare are several pros and cons. On top of the muscle-less painful post-op (short term issue and I advise patients to not consider this), less breast tissue visualized on mammography (has not translated into a medical problem for patients in terms of delayed diagnosis of breast cancer, etc.), possibly higher incidence of capsular contracture although this is very murky, no risk of distortion with activity like the implant jumping when flex pecs.

Below muscle-more painful, more tissue visualized on mammo, may have lower capsular contracture rates, can have distortion of implant with pec muscle activity, implants may drift down and out over time.

Fuurther considerations-are you a body-builder? They prefer on top of the muscle. Another-do you need a breast lift at same time? If you do I prefer on top of the muscle as long as your tissues are not too thin.

Large Breast Implants after Surgery

Patients are often unaware that swelling plays a major role in the postoperative course after breast augmentation. It is important to maintain a good rapport with your surgeon and allow the swelling to subside before embarking on any plan for definitive size reduction.

Breast Implants in NYC – Incisions

Nobody wants visible incisions and they are, in general, pretty well hidden. But dependin on the type of implants you have, the location of the incisions and specifics of your skin type, it is not realistic to expect them to be completely hidden.

The most common incisions are periareolar (more specifically and accurately infra-areolar, since they are around the lower portion of the areola), infra-mammary (under the breast), axillary (armpit) and umbilical (belly-button). The last is available for saline implants only and the first two are the most commonly used.

Size wise I think the implant alone will do the job, but the need breast mound will not be in the old place due to the stretching of skin from the pregnancy. To correct that would require a breast lift also .
