Breast implants and breast augmentation in Maryland should have no effect on your ability to breast feed. The milk production gland is not damaged and the ducts to the nipple are not damaged during the surgery. Keep in mind though, that some women are unable to breast feed – regardless of implants.

So, if you cannot breast feed after implants, it is likely that you would have ahd difficulty even without the surgery.

Lift or no lift

A very common question in my practice. I see many patients who had oversized implants just to avoid a lift. This will cause long term problems with a higher than usual chance of requiring revision surgery.

Your best option is to have a lift with an implant that will fit your breast diameter.

Choosing a Breast Implant Size

So many women are perplexed by the choice of implants. Personally, I don’t recommend selecting an implant based on adding volume to a bra since an implant inside your body contours very differently from peas, etc placed in a bra on top of your skin.

I recommend that my patients find photos of breast contours that they like to help me decide during surgery which implant combines the optimum aesthetic shaping and size for each individual patient. Having said that, looking at your photos and your measurements I’d lean toward the 300 cc size for you (but again would only make the final decision in surgery based on how the test implants contour and appear).

You’ve choosen a very large implant. You will be big and have a whole different life experience from that point on. I think your plastic surgeon was correct in recommending something in the 400 range. I suspect it will make you a full C or even a D.
This size looks really good. 600 cc implants are going to have everybody looking and talking about you. 400 cc range will have everybody saying how great you look.

To determine what would be both best and realistic for you, you need to be seen in person by a board-certified plastic surgeon who will be able to guide you through the process.

Please also keep an open mind to a having a breast lift with implants as well.

Breast Implants via Tummy Tuck Incision

While it is technically possible to place breast implants through the tummy tuck plane, this is a very difficult way to control the shape of the breast pocket, which is critical in creating your final result.

My personal opinion is that the inframammary incision provides excellent access the the submuscular pocket, is well hidden, and minimizes sensory loss to the nipple.

Breast Augmentation Approach

You may be also be a good candidate for the breast fold or even the armpit approaches. I enjoy the Transaxillary approach with an endoscopic camera. The scar is totally hidden and dissection is done while watching the movements on a screen for exact precision.

It’s a great option if your body allows for it.

Can breast implants in Maryland be done post reduction mammoplasty?

Patients who have had previous breast reduction surgery are able to undergo breast augmentation surgery in Maryland; either type of breast implant (saline or silicone gel) can be utilized, depending on the patient’s anatomy, goals, preferences…

Sometimes, patients in your situation (recent weight loss) also benefit from additional breast lifting surgery; this procedure can usually be done through the previous breast reduction incision lines.

You may wish to post photographs for more precise advice. Otherwise, seek consultation with board-certified plastic surgeons to discuss your goals. Ask to see lots of examples of their work and communicate your goals clearly as well.

To lift or not to lift…

As you can see here, even online, you will get many different opinions all from board certified plastic surgeons. We have different experiences, abilities, and philosophies when it comes to plastic surgery.

This is why it is art and not just science. If this was a factual test, like 2+2=4, you would get all the SAME answer. But this is much more complex a question. My impression of the pics you show, is that you have some sagging and volume loss, but it’s not terrible.

I have many patients who would be perfectly happy with your degree of sagging and some patients who would not tolerate this. it’s a very personal decision. However, take a look at your nipple position.
it is roughly halfway between the armpit and elbow crease, and it “points” forward, not to the ground. From the side view, you can see the nipple position is just slightly above the lateral breast crease.

In MY experience, this is a patient who would most likely be happy with just the implants in MD alone. you will still have some mild sagging but it will look natural. The left nipple is slightly lower than the right, so you may want to have a correction of the nipple position ( if you opt to not have the lift ) I will agree with the others to keep the position UNDER the muscle.

Further, you can always return to have the lift at a later time, if you deem it necessary. You don’t burn and bridges by doing this in stages vs at one time.

How long do implants last…do they need to be changed every 10 years

It is true that implants will have some eventual wear and may ultimately need to be replaced. It may take 10 years, or longer for this to occur.

If your implants in MD are in good position and you are happy with them without sign of a rupture or implant related problem they do not need to be replaced.

I ask all of my breast implant patients to follow up with me routinely so I can monitor their results and help them determine if there are any implant related concerns along the way.

Augment only

You should be able to achieve a very pretty enhancement with implants only. Please consult with a board certified plastic surgeon and be sure that his or her sense of aesthetics aligns with yours.

Is it possible to place implant under muscle, when you go through the areola?

Always best to see a few private practice boarded Plastic Surgeons FIRST than ask all your questions. The answer is YES!.

Placement of Breast Implants

There is not a “right” answer that fits for all patients. Breast implant placement depends on your anatomy, tissue thickness, and desired outcome. In general, I prefer to place the implants under the muscle in Maryland. I feel that this produces the most natural look to the breast.

Placing the implant under the muscle also increases the amount of your own tissue between your skin and the implant, which make the implant less palpable.

Too early

It is too early to evaluate the outcome. You have swelling and are just getting used to the new volume. Give it 6 months. Most people that have concerns about size early after surgery are happy at 6 months.
You are not at a greater risk

When it comes to breast surgery you are asking the right questions. Being that you are 50 it is important to get a mammogram before getting your breast implants. That being said, no, there is no reason why breast implants would increase you risks for getting breast cancer.

The two factors that influence breast cancer the most are gender and age. Mammograms are highly specialized machines and can see the breast tissue even with implants.

Should I have saline or silicones ?

Most of us would recommend cohesive silicone gel implants in MD although an option for you might be to consider the Ideal Saline Implant. The latter has baffles and tends to have less rippling and a more natural feel than regular saline implants.

Wait six months

I usually recommend patients wait for at least six months before considering any cosmetic breast surgery. This is for your safety.

Techniques to aid implants in settling

At this point during your recovery, aggressive downward massage will help the implants settle into the pocket. Make sure to relax the muscle as you massage the breasts, otherwise it will defeat the whole purpose of the massage.

Using an ace bandage or band to put pressure on the implants will also help, but the massage will give you a more significant result more quickley. Massaging and manipulating the breasts also prevents the chances of encapsulation.

If you do not notice any change within 4-6 months, consult with your plastic surgeon to discuss possible revisionary surgery if necessary.

You Can Fly With Breast Implants

Patients are frequently concerned about the safety of breast implants when they fly at high altitudes. These concerns are related to a large number of urban myths about exploding breast implants in Maryland. It’s safe to say that breast implants don’t explode at high altitudes and are extremely safe.

There’s no reason not to fly when you have breast implants.

Breast augmentation and exercise

In general, athletic women tend to be individuals with low body fat levels and little breast tissue. Body fat and breast tissue tend to help camouflage the breast implant. For this reason, placing the breast implant under the muscle, which in most cases means partially under the muscle since the muscle does not completely cover the breast implant, is the preferred location.


There are some trade offs between over and under the muscle. There is the possibility of breast implant movement with muscle contraction. I have never actually has a patient complain of that problem and I hove performed breast augmentations on a number of athletic women some of whom compete.

Under the muscle helps to conceal rippling and wrinkling which can be an issue with saline implants primarily. Visibility is a problem all the time if it is present. Breast implants in Maryland placed under the muscle tend to stay softer over time and this is especially true for silicone implants, which are the implants we use primarily and which I would recommend to you.

Most athletic women in my practice choose conservative sized implants, which not only enhance their overall appearance naturally, they do not interfere with their exercise activities. and best of luck.

What kind of breast surgery would be best for me? 22, no kids, on the heavier side.

I’m sorry to hear about the issues that you’ve been having. For the acne I would recommend seeing a dermatologist. They will give you a comprehensive plan to get the acne under control. It appears that you would benefit from a lift and an augmentation to improve the shape and to increase the size of your breasts.

In my office, I would offer a patient with your anatomy, a two stage procedure. The first stage would consist of a breast lift. The second stage would include an augmentation with either a silicone implant or fat transfer.

Discuss the pros and cons of implants or fat with your surgeon during your consultation. My preference is to separate the two procedures when there is significant sagging, because I find that the revision rate is decreased.
Make sure to see board certified plastic surgeons.

What type of breast surgery is the best option for an athletic 35 year old?

You do not look like you will need a periareolar breast lift, as your nipples are not particularly low and you do not appear to have too much breast overhang. The implants alone should fill your breasts out nicely.

As to size, your surgeon should have you try on sizers to determine what volume you would like to add to your breasts, and you should show him pictures to communicate how much upper pole fullness you are hoping to achieve.
